Court Street: Open to individuals with an intellectual or physical disability

Applicants must:
-Be age 18 years or older and capable of making decisions about their own health
-Be eligible for health services funded by a government of Canada
-Have a grievous and irremediable medical condition (including an illness, disease or disability)
-Have made a voluntary request that was not influenced by external pressure
-Give informed consent after having been informed of the means that are available to relieve their suffering, including palliative care
