HOBSCOTCH Provided by: Epilepsy Association of the Maritimes - AKA: Epilepsy Association of Nova Scotia (former name)

Provides an 8-session virtual program for adults with epilepsy experiencing cognitive challenges. Working one-on-one with a certified cognitive coach, participants learn practical strategies to improve memory, thinking skills, and daily functioning, helping them feel more confident and in control. Includes:

Education to teach participants the basics about memory and how seizures and variables related to epilepsy may impact their memory and cognition
Self-awareness training to teach participants to be aware of the multiple internal and external influences that may impact memory and thinking
Memory strategies to help improve day-to-day cognitive challenges. The HOBSCOTCH Memory Toolbox provides tried and true strategies to help participants feel more in control
Problem Solving Therapy (PST) to help you identify memory-related challenges in their daily life and find solutions to manage these problems
Mindfulness, including progressive muscle relaxation and deep breathing exercises, to help reduce stress and anxiety

Home Care - COACH Program Provided by: Health PEI

Provides in-home support to frail seniors with complex health needs to help them live at home longer, return home from hospital stays sooner or transition to long term care.

The program is offered jointly by professionals from the Home Care, Primary Care and Geriatric Programs who assist seniors in learning to better manage their health and make decisions that positively impact their quality of care and life.

Provides in-home services for people who require short or long term support to remain in their homes safely. Depending on individual needs, may include:

Nursing care: Health monitoring, IV therapy, injections, wound care, and patient teaching
Home support: Assistance with daily activities like bathing and dressing
Social work: Individual and family counselling for illness, loss or end-of-life issues
Dietitian services: Nutrition assessment and education
Physiotherapy: To maximize independence, function and mobility
Occupational therapy: Support for daily living, special devices, equipment or modification to home or workplace
Long term care: Assessment for nursing home admission
Palliative care: Comfort and support to live fully until the end of life
Adult day programs - enriching social activities for seniors

Provides specialized nursing and personal care services to people who can no longer live on their own, with family or with home care supports. Services are provided in both public and private long-term care nursing homes and include:

Remote Patient Monitoring Program Provided by: Health PEI

Provides a free at-home monitoring program for PEI residents living with heart failure or Chronic Obstructive Pulmonary Disease (COPD). Patients will receive monitoring equipment that will send vital signs directly to an RPM nurse.

Program helps patients to:

Learn to manage their chronic disease from the comfort of home
Avoid unnecessary trips to the hospital
Catch problems before they become an emergency
Become more involved in managing their health and quality of life

Support Match PEI Provided by: Prince Edward Island Association for Community Living - AKA: ACL; PEIACL; PEI Association for Community Living

Helps connect families with support workers able to provide home support on either a volunteer or fee-for-service basis. Families and support workers are added to a registry database which can be searched to find possible family/worker matches. The types of services offered by support workers may include:
