Eligibility:Open to those with a palliative diagnosis and their caregivers or someone living with a life limiting condition
Fees:No fees
How to Access:Self-referral through calling or emailing the regional offices, or referral from a health care team
Documents Required:Verification of diagnosis through an assessment by Hospice PEI coordinators
