Eligibility:People who have been injured on the job * Claims must be submitted with 6 months of the date the accident occurred
Fees:No fees
How to Access:File a claim online, by email, or by mail (forms available online) * Report the injury to your employer and seek medical attention immediately (Inform your healthcare provider that the injury is work-related) * Submit your signed Worker's Report within 6 months of the incident
