Description:	Helps identify early identification of risk for falls. Includes: Fall risk screening by a physiotherapist Home exercise program Home safety information Education to prevent falls and common foot issues Eight-week follow-up and reassessment Travel clinics may be available in Montague, Summerside and O'Leary.
Eligibility:	Individuals 50 years and older who have experienced or are at high risk of falling or have changes in sensation in feet
Fees:	No fees
