Helps youth living with type 1 diabetes as they transition from high school to post-secondary education and beyond by providing financial, social, emotional, and informational support.

Provides in-person and virtual workshops for youth living with Type 1 Diabetes (T1D) and their caregivers who are preparing to transition from pediatric to adult care. Support includes:

  • Transition guidance
  • Understanding the differences between pediatric and adult care
  • Preparing for adult care appointments
  • Information about patient rights and advocacy
  • Finding their ideal healthcare team and more

The program consists of three components:

  • Part one: Youth Workshop, Adulting with Diabetes
  • Part two: Online Support Forum for Youth
  • Part three: Caregiver Workshop

Provides an opportunity to students living with type 1 diabetes to become a mentee or a mentor and have a one-on-one mentorship opportunity.

Provides online community to help students living with type 1 diabetes to connect with peer to support each other during their transition to post-secondary school.

Provides a scholarship to students living with type 1 diabetes as an additional financial support for their transition years while recognizing their achievements.

Application opens in January and closes in March each year.
