Eligible applicants must:
-Have permanent impaired mobility
-Have a significant and persistent movement impairment
-Have an up-to-date medical record (Please download the medical report and have it filled out by your doctor before proceeding with your request)
-Be able to care for your dog

The youth with ASD must:
-Be between ags 5-21 years
-B showing an interest in dogs;
-Have significant needs at home and when going out(For example, they have needs that cannot be met by a companion dog)
-Demonstrate certain skills related to the management of a dog or have the potential to acquire the required skills
-Be a resident of Canada

Eligible applicants must:
-Be 11 years old and older
-Be blind, deaf-blind, have low vision or a visual and mobility impairment
-Have a visual and medical report confirming your disability (Please download medical report and have it filled out by your doctor before proceeding with your request)
-Be able to interact with our dogs
-Be in good physical shape (except for persons with a visual and mobility impairment)

CNIB member ages 16 years and older who meet specific criteria as assessed by an orientation and mobility instructor

Must be: Registered legally blind * 16 years and older * Permanent resident of Canada * In reasonable physical health to work with and train a guide dog

Individuals 16 years and older who are diagnosed with Type 1 Diabetes with hypoglycemic unawareness
