Adult amputees ages 18 years and older residing in Kanata / Canada

18 years and older * Resident of PEI * Have a valid PEI Health Card

18 years and older * Resident of PEI * Have a valid PEI Health Card

Resident of PEI, New Brunswick or Nova Scotia living with epilepsy or their families * Must travel out of the province to receive a treatment

Applicants must:
-Be permanent PEI residents as defined by the Health and Dental Services Cost Assistance Act
-Have a valid PEI Health Card
-Have filed their most recent income tax return in PEI

PEI residents diagnosed with ALS * Must register (for free) as an ALS Society client

Eligible PEI residents who:

-Have a valid PEI health card
-Have no or insufficient insurance coverage (or cannot afford their co-pay)
-Have an appropriate diagnosis from a physician or nurse practitioner (see above referenced list of diagnoses)
-Have not previously received pelvic floor physiotherapy for this conditionnot previously received pelvic floor physiotherapy for this condition

People who have been injured on the job * Claims must be submitted with 6 months of the date the accident occurred

Residents of PEI * Must travel out of the province to receive a lung transplant

PEI residents diagnosed with ALS * Must register (for free) as an ALS Society client * Must be unable to get funding for equipment through other sources such as Provincial Palliative Care, Veterans Affairs Canada, Red Cross, or private insurance
