Information
Eligibility:
Birth certificates are issued to:-The person named on the birth certificate-Parent as long as their name appears on the official birth registration-An individual authorized in writing by anyone named on the birth certificate-A person requiring a certificate for administration of the estate (when the individual named on the certificate is deceased)-A lawyer acting anyone named on the birth certificate providing written authorization-In response to a court order

Apply online or download "Application for a PEI Birth Certificate" form and submit to a Vital Statistics office in person, by mail or fax
