Travel Treatment Fund

Service Description

Provides low-income individuals living with cancer an annual travel allowance to help cover the costs of travelling to cancer related appointments. One travel allowance is granted per year per person.

Information

Eligibility:
Must: Be undergoing active cancer treatment * Earn below the set threshold * Be the person with cancer (funding not for a third party) * Not be on Social Assistance as financial assistance is available through that program * Have at least 10 appointments per year related to treatment OR live more than 200 km away from the treatment centre

How to Access:
Have the application form completed by a health professional and return it by mail, fax, or email
Documents Required:
Most recent Notice of Assessment from Canada Revenue Agency (CRA) for all members of the family unit over 18 years of age * If applicant or their partner's financial situation has changed during the year, proof of current family unit income is required * Signed Direct Deposit Enrollment Form with a void cheque (if applicable)
