Provides free dog guides and required training for people with epilepsy. The dog guides are capable of activating an alert system or barking for help in the event of a seizure.

People who have epilepsy and are 10 years and older

Complete a program application form, neurologist form, and a physician form (other specialist reports as applicable) * Available online or by mail
