Service Description
Provides one free eye exam and, if required, one free pair of glasses during a child's kindergarten year.
Information
Eligibility:
Children in their kindergarten year * Parents with or without private insurance can access the program for their child
Fees:
Yes * Parents with private insurance may need to pay the optometrist up front then submit a form to Health PEI for reimbursement of costs
How to Access:
Parents without private insurance: Book an appointment with an optometrist * You will be able to complete and sign a Health PEI Declaration Form when you go to the appointment and the cost of the appointment and glasses (if needed) will be free * The form is also available online for those who wish to print it out in advance and bring it with themParents with private health insurance: Book an appointment with an optometrist * Bring your private insurance information with you to the appointment * If the optometrist is unable to bill your insurance provider directly, you will have to pay up front and be reimbursed by submitting your claim to your provider and submitting a completed Application for Co-Pay Reimbursement (available online) to Health PEI
