Provides stroke survivors and caregivers support in getting the help they need from health care professionals and community resources. Includes:
- Connecting you with health care professionals who can provide supports
- Helping you find and access community resources
- Providing your with access to informative booklets, websites and support groups
- Answering questions about the stroke recovery process
