Service Description

Assists with the costs associated with approved insulin pumps and supplies.

The program covers up to 100 percent of costs. The amount depends on annual household income, private insurance coverage ,and device costs. See website for full details.

Notes: Application deadline is July 1 each year. Existing recipients must reapply by the deadline to keep the current coverage.

Information

Eligibility:
PEI residents of all ages living with type 1 diabetes who:

Reside in Epekwitk / PEI
-Have a valid PEI health card
Have filed most recent income tax return in Epekwitk / PEI
-Meet certain medical criteria

Yes * Applicants must be able to pay their portion of costs
How to Access:
Complete self-assessment form Individuals with private insurance need to contact their provider to determine what percentage it covers for insulin pumps and supplies * Use Insulin Pump Payment Calculator to determine break downs of payment details * Contact health care provider to complete an intake assessmentSee website for full details
