Peer Support Program

Service Description
Trained peer support workers provide individualized plans to help clients to go through their mental health journey. Includes:
- Crisis intervention
- Hospital to Home program
- One-on-one session
- Peer support groups
Information
Eligibility:
Adults with a primary diagnosis of mental illness and are age 18-65 years at the time of application * Can be struggling with or in recovery from mental illness
