Service Description
Trained peer support staff provide one-on-one support for individuals who are experiencing mental illness.
Information
Eligibility:
Adults with a primary diagnosis of mental illness and are age 18-65 years at the time of application * Can be struggling with or in recovery from mental illness
Fees:
No fees
How to Access:
Call, email or drop in for a referral form to complete * Can also book a tour of the program prior to or after completion of the referral form
Documents Required:
Referral form must be completed by one of the following: -Self-referral-Referral by another CMHA program-Referral by a health care professional such as: Psychiatrist * Doctor * Nurse * Mental Health Counsellor * Social Worker-Healthcare provider (ex. psychiatrist, doctor, nurse, mental health counselor, social worker, etc.
