Service Description

Provides specialized nursing and personal care services to people who can no longer live on their own, with family or with home care supports. Services are provided in both public and private long-term care nursing homes and include:
  • 24-hour nursing care
  • Room and board
  • Personal care
  • Medical services

Information

Eligibility:
People who can no longer live on their own, with family or with home care supports * Must be age 60 years or older and be assessed by Home Care to require a nursing level of care * Individuals under age 60 years may be considered for long-term care if assessed as needing nursing level care and no other reasonable alternatives exist
