Individualized Programs

Service Description
Clients are assigned a worker to assess their unique needs and help develop a case plan to meet those needs. After consultation and assessment, clients are connected with community resources and programs to help with the following types of needs:
- Community based resources
- Fitness and recreation
- Socialization
- Life skills

Eligibility:
Open to people who are deafblind, have a sight or hearing disability paired with another disability * Other cases may be considered * Support also provided for families of individuals
