Service Description

Anyone who received a COVID-19 vaccine in PEI can access a copy of their immunization record.

The record features a unique QR code and authenticated record of vaccination that includes the date(s) of vaccination and kind of vaccination received.

Individuals can request a vaccine record on behalf of a family member, provided they reside in the same household as the requestor. Parents/guardians do not have to live in the same household to request a copy on behalf of their children.

Island residents who received a COVID-19 vaccine outside PEI can have the information added to the PEI Immunization record. A PEI Health Card is required to complete the online form.

Processing time is generally 24-48 hours.

Previously this COVID-19 immunization record had also been called a PEI Vax Pass, which permitted fully vaccinated individuals to gain entry to most public events and non-essential services. A Vax Pass is no longer required in PEI.

Medical Exemptions: Only medical exemptions approved by the PEI Chief Health Office are accepted. Exemptions are granted for extremely rare circumstances where an individual cannot receive a COVID-19 vaccine due to an allergy to vaccine ingredient(s) or having experienced a serious medical event after receiving a first dose of the vaccine. Primary care providers (physician or nurse practitioner) must submit a medical exemption request form on behalf of their patient to the Chief Public Health Office. The request will be reviewed and, if the patient meets the criteria, a Medical Letter of Exemption will be sent to the physician/nurse practitioner to provide to their patient. Substitute letters or health care provider notes claiming medical exemption are not accepted.

Tip: The portal works best when using Chrome on a PC and Android device and Safari on Apple devices.
