Glucose Sensor Program

Service Description
Funding available to help with the cost of glucose sensor technology (Continuous Glucose Monitoring (CGM) or Flash Glucose Monitoring Systems) to monitor sugar levels in individuals with diabetes.
Supplies that are eligible for coverage include:
Eligibility runs from the start date noted on the approval letter/email to June 30th each year. Annual applications are required and must be received by July 1st. Applications can be sent in up to three months before the deadline.
Supplies that are eligible for coverage include:
- Abbott Libre or Libre 2 sensors
- Dexcom G6 sensor
- Medtronic Integrated (CGM) Guardian Sensor (3) or Guardian Connect CGM

Eligibility:
-Permanent residents as defined by the Drug Cost Assistance Act
-Have a valid PEI Health Card
-Have filed most recent income tax return
-Have been diagnosed by a medical practitioner or nurse practitioner as having diabetes
-Are reliant on an insulin pump or multiple daily injections of insulin (3 or more)
-Have a signed Special Authorization from your medical practitioner, nurse practitioner or certified educator
-Have a valid PEI Health Card
-Have filed most recent income tax return
-Have been diagnosed by a medical practitioner or nurse practitioner as having diabetes
