An online form that allows the public to contact Service Canada to request a phone call, book an appointment or apply for a Social Insurance Number.

Phone calls can be requested for questions about:

• Employment Insurance (EI)
• Canada Pension Plan (CPP) / Canada Pension Plan Disability
• Old Age Security (OAS)
• Guaranteed Income Supplement (GIS)
• Veterans Affairs Canada services
• Social Insurance Number (SIN) information
• Ukraine immigration measures: financial assistance
• Record of employment on the web (ROE web)
• Canadian Dental Care Plan (CDCP)

Service Canada will respond within two business days by phone. If a request cannot be completed by phone, you will be offered an in-person appointment at a Service Canada Centre.
