Service Description
Helps identify early identification of risk for falls and early identification of foot ulcer risks in people with diabetes. Includes:
- Fall risk screening by a physiotherapist
- Home exercise program
- Home safety information
- Screening for common diabetic foot issues
- Education to prevent falls and common foot issues
- Eight-week follow-up and reassessment
- Footcare by a licensed practical nurse for those unable to access community footcare

Eligibility:
Individuals 50 years and older who have experienced or are at high risk of falling or have changes in sensation in feet
