AKA: Diabetes Active Steps for Health

Service Description

Helps identify early identification of risk for falls and early identification of foot ulcer risks in people with diabetes. Includes:
  • Fall risk screening by a physiotherapist
  • Home exercise program
  • Home safety information
  • Screening for common diabetic foot issues
  • Education to prevent falls and common foot issues
  • Eight-week follow-up and reassessment
  • Footcare by a licensed practical nurse for those unable to access community footcare
Travel clinics may be available in Montague, Summerside and O'Leary.

Eligibility:
Individuals 50 years and older who have experienced or are at high risk of falling or have changes in sensation in feet
