COVID-19 Vaccine - Reporting Side-Effects

Service Description

Individuals should report any serious side-effects or reactions (adverse events) after receiving a COVID-19 vaccine.

If the following symptoms are experienced, they are a sign of an allergic reaction
CALL 9-1-1 right away:
  • Hives (bumps on the skin that are often very itchy)
  • Swelling of the face, tongue or throat
  • Difficulty breathing such as coughing or shortness of breath

To report side effects, contact the location where you received the vaccine. They will complete and submit a report to Public Health. If unable to do so, contact one of the following to report side effects:
