Cancer Patient Navigation Program

Service Description

Provides support to people who have been diagnosed with cancer and their families. The Cancer Patient Navigator can help to:
  • Explain what to expect during appointments and treatments
  • Find answers to questions about cancer screening and genetic testing
  • Find answers to questions about care plans
  • Provide brief supportive counselling
  • Connect patient with other professionals and services that provide support throughout cancer care including : Social workers * Dieticians * Home care * Spiritual care * Couselling * Tobacco cessation support * Breast prosthesis program
  • Help prepare for and provide support during out-of-province travel for cancer related medical appointments and procedures
  • Connect to resources to help with financial needs and expenses for medications, travel or medical supplies
