Cancer Patient Navigation Program

Service Description

Provides support to people who have been diagnosed with cancer and their families. The Cancer Patient Navigator can help to:

Explain what to expect during appointments and treatments
Find answers to questions about cancer screening and genetic testing
Find answers to questions about care plans
Provide brief supportive counselling
Connect patient with other professionals and services that provide support throughout cancer care including : Social workers * Dieticians * Home care * Spiritual care * Couselling * Tobacco cessation support * Breast prosthesis program
Help prepare for and provide support during out-of-province travel for cancer related medical appointments and procedures
Connect to resources to help with financial needs and expenses for medications, travel or medical supplies
