Access Personal Health Records and Request Changes

Service Description
Individuals can access their personal health information and request changes to those records.
Request a Copy of Hospital Medical Record:
To access personal health information an individual must contact the Heath Records Department at the health facility where care was received.
If care was received in a hospital, individuals must contact the Health Records Department directly.
Individuals will be required to sign a Consent to Release Information form. There may also be a fee for copying documents depending on the number of pages requested.
Request a Correction To Personal Health Information:
Corrections to health care records can be requested through the Manager of Health Privacy and Information Access. Individuals can submit a request online or complete the Request to Correct Personal Health Information form which can be submitted via email or regular mail. Requests must include detailed descriptions of the information believed incorrect and what correction is needed.
Request a Correction To Personal Health Information for Someone Else:
Parents or guardians can request a correction to personal health information for another person if they have the legal authority to act on their behalf. Documentation will be required to prove that authority.
Requests to update an address or phone number do not require a Request to Correct Personal Health Information form. Notify PEI Medicare by calling 1-800-321-5492.
There are no fees for requesting a correction in a personal health record.
Request a list of hospital or clinic visits
Individuals can request a list of their hospital or clinic visits by submitting a request to the PEI Medicare by calling 1-800-321-5492. A consent may be required.
