Note: As of November 1, 2024, retired volunteer firefighters with at least 15 years of service will receive free registration and license plates for two vehicles and free driver's license renewal.
Individuals can review their records, obtain a copy, and request changes to them.
Request a Copy of Hospital Medical Record:
To access personal health information an individual must contact the Heath Records Department at the health facility where care was received.
If care was received in a hospital, individuals must contact the Health Records Department directly.
Records held by a family doctor or nurse practitioner can be accessed by contacting the office directly.
Individuals will be required to sign a Consent to Release Information form. There may also be a fee for copying documents depending on the number of pages requested.
Request a Correction To Personal Health Information:
Corrections to health care records can be requested through the Manager of Health Privacy and Information Access by submitting the completed form online, by mail, or via email
Request a Correction To Personal Health Information for Someone Else:
Parents or guardians may request corrections to someone else's health information if they have legal authority, with documentation required.
